Doing that doesn't just clear the cache, it also erases system and game patches. Holding A for a few seconds while a disc is loading will clear the cache as well, but without deleting all your patches.
Does that work for all games? Are you sure it works for GTA IV?
It won't work for games that don't use the cache. But any game that does, holding A while it's loading up will clear it. Or so I've been told.
